POSITION TITLE: PATIENT CARE REPRESENTATIVE

FSLA: NON EXEMPT

Under the direct supervision of the Charge Nurse, incumbent is responsible for monitoring and screening of all telephone calls for the Clinical Department.

You Will Need:

  1. High School Diploma/GED.
  2. Certified/Registered Medical Assistant preferred; Cert Nursing Assistant at minimum
  3. Computer literate.
  4. Two years experience as a customer service or public relations representative in clinic or
  5. hospital, preferred.
  6. Strong knowledge of medical terminology.
  7. Strong interpersonal/telephone skills.
  8. CPR (provided by SCHC within 6 months of hire)
  9. Bilingual (Spanish/English) preferred.

Job Duties:

  1. Monitor and respond to all incoming calls for medical dept prioritizing urgency and route to appropriate staff for action
  2. Assist site /provider's nurses with medical phone encounters, relaying instructions and messsages to patients
  3. Document all phone encounters and actions taken
  4. Forward all calls regarding assessment, symptoms and judgements, emergency care or medical advice to charge nurse, triage nurse, and POD nurse at clinical sites
  5. Review request of pharmacy orders for medication and refills, per Call Center Policy #T-150
  6. Assist patients requesting appointments with their PCP
  7. Assist patients with referral requests
  8. Other duties as assigned
