POSITION TITLE: CASE MANAGER
FSLA: EXEMPT

Under the direct supervision of the Medical Director and indirect supervision of the QI Manager, incumbent will develop, implement, and evaluate individualized patient care plans. Will act as an advocate for patient welfare, and serve as a liaison between patients, their families, and healthcare providers. Incumbent will also assist in assuring HIPAA, AAAHC and HRSA guidelines are followed. Employee shall function in compliance with all organizational policies and procedures.

You Will Need:

  • LPN/RN with healthcare expreience in nursing processes.
  • Knowledge of HEDIS and the use of HEDIS gap reports.
  • Knowledge of helathcare regulations, QI/RM/UR programs, audits and manager care plans.
  • Quality monitoring and critical thinking skills
  • Ensures communication with staff, patients, and external contacts occur with courtesy, tact, and a posotive attitude.
  • Computer literate
  • American Heart Association CPR REQUIRED
  • Bilingual Engliosh/Spanish preferred
Job Duties:
  • Completes initial and ongoing documented telephone contact with patients/family.
  • Teaches chart review to staff and nurses.
  • Provides ongoing outreach and case management to a caseload of identified SCHC patients. Through record review and communication with PCP, encourage patient compliance.
  • Advocates for individuals and their family members to include problem solving issues related to the healthcare system and within the community as needed.
  • Provides continuity of care as liaison to hospital discharge/case management staff and insurance representatives to assure follow up post emergency room visits/hospital admission discharge.
  • Consistent application of the risk management program to include reporting all risks to supervisors to ensure compliance of all safety programs company-wide updating policies as needed.
  • Keeps all matters related to the organization confidential in compliance with confidentiality policy.
  • Reviews and assists in management of patient complaints/compliments presented.
  • As record custodian for the organization, performs accurate reviews of medical records.
  • Communications with the professional liability insurance carrier under the supervision of the CEO.
  • Responsible for depositions, subpoenas and direct communication with attorneys with periodic review of all litigation.
  • Stays current with changes in the health care delivery system to provide staff education in risk management within thirty (30) days of employment and thereafter.
  • Assumes responsibility for work performance
  • Works closely with Quality Improvement in promoting performance improvement activities and participates in special projects/committees.
  • Ability to provide own transportation in the performance of assigned duties
  • Other duties as assigned
